SMILE

A laser technique without a flap: how it works, and its strengths and weaknesses compared with LASIK.

SMILE is a technique that has come into use in recent years.

In this technique, unlike LASIK, no flap is lifted from the cornea for the laser to reshape the tissue underneath. Instead, cuts are made in the cornea in two planes and the piece of tissue between them is removed from the cornea.

The technique can be used for hyperopia, astigmatism and myopia. In myopia, however, the corneal thinning it causes only equals that of LASIK when the prescription is high (above about −5 to −6). At lower prescriptions the cornea is thinned more than with LASIK, which is not desirable.

Over the years, with changing laser and computer systems, SMILE may prove itself. For now, though, it is a young technique compared with LASIK, whose results are known over 30 years.

In addition, if some prescription remains after SMILE, the second touch-up treatment cannot be done with SMILE; it can only be done with LASIK.
